Some of the things I've seen which lead to the biggest threats to malpractice insurance involve 1) improper or insufficient patient identification, and 2) non-compliance with universal precautions. Both can be rampant, in some places, and are SO easily avoided with simple measures! These are all great tips for the health care industry, and for patient awareness, as well. A simple one, but a lie nonetheless. So when my treatment didn't go so well, the first thing that came to mind was to question the doctor's ability, and intentions.
